Position: Patient Navigator
Program: Akoka’tssini Mental Wellness – Inii Healing House
Classification: Full Time – Term Position
The Patient Navigator will work directly under the supervision of the IHH Program Leader. The Patient Navigator will combine health knowledge, communication, and administrative skills to effectively assist in the delivery of care services. The Patient Navigator will build relationships and networks with organizations that can offer supplementary support to patients. Patient-focused services will offer compassionate and empathetic care to all patients through a trauma-informed care approach. Performs all duties and responsibilities in accordance with the Piikani Nation’s Personnel Policy & Procedures.
About Us:
Aakom-Kiyii Health Services is located on Piikani Nation. It offers a full range of community health and Wellness to the Piikani people and their families. This includes traditional Wholistic approaches to Mental Wellness, health promotion, disease prevention, curative care, rehabilitative care and social resource advocacy.
Duties include (but are not limited to):
- Assist and guide residents at IHH to ensure they are receiving proper support and solutions in our healthcare system
- Collaborate with appropriate team members to ensure comprehensive care
- Follow up with residents to monitor progress and address any ongoing needs
- Advocate on behalf of residents to remove barriers and improve access to services
- Maintain accurate and up-to-date resident records
- Provide educational resources and tools to help residents become more self-sufficient
- Completes required reports and documentation, competent in using electronic documentation and statistical record keeping, supply requisitions, resident billing approval forms and workload statistics.
- Performs clerical/administrative duties such as follow up phone calls, booking appointments, creating charts, organizing and filing charts, relaying messages, and recording data such as resident contact time as required.
- Transport residents and equipment and/or supplies to appointments as required.
- Respects Blackfoot traditional practice protocol(s) when engaging with Piikani members
Minimum requirement:
- Minimum requirement: Health Care Aide Certificate
- Post-Secondary Diploma/Degree in Health Sciences would be an asset
- Two (2) year of related experience
- Current First Aid/CPR is an asset
- Valid driver’s license
