Mission Statement
Home Care Program Objective – Home Care nursing will assist people who have health problems to remain living in their own home in the community as long as it is safely possible. Home Care services are provided in an efficient and confidential manner.
Referrals
Home Care services are provided to clients based on identified needs through a client assessment by a Home Care Manager. You may refer yourself, or you may be referred by a family member, doctor, hospital, or friend

Intake/Screening Involves:
- Initial Identification of need.
- Identification of eligibility for service
- Assessment of the potential client
- Identify the urgency of the need
- Identify of the priority for service
