Position:          Community Health Representative

The CHR works in close cooperation with our health and allied professionals and community agencies to promote health and prevent diseases for clientele of Piikani Member across the life cycle. Performs all duties and responsibilities in accordance with the Piikani nations, personal policies, and procedures. 

 

 

Role & Responsibilities  

 

  • Provides interpretations in all areas of health in a cultural appropriate context, which includes physical, spiritual, emotional and intellectual factors 
  • Refer clients to appropriate healthcare and all professional professionals as needed 
  • Visits clients in the home for ongoing monitoring and assessments as indicated by needs or at the request of other healthcare professionals. Such visits may include counselling and glucometer and other appropriate routine screening procedures for which the CHR has been trained. 
  • Assist other health care professionals in obtaining health, histories, and related information 
  • Arrangements and coordinates appointments and where appropriate arrangements for transportation for clients. 
  • Work cooperatively with all programs of AKHS, making follow-up visits and the request of the CHNs 
  • Promotes awareness of Canadian Food Guide, translating the information into cultural appropriate context to assist client, clients, families, and community groups in meeting their nutritious needs. 
  • Liaison, on clients, family, or community’s behalf with other CHR programs, community partners, physicians and health, education, social services, and other community or first nations agencies. 
  • Provide information, education, counseling, contact, tracing, and follow up for communicable disease in conjunction with health protection and public health, nursing staff under the direction of the nurse in charge and/or medical officer of public health. 
  • Provides, development and translations health, education and health promotion materials into the appropriate culture context. 
  • Works closely with the First nations, health, promotion, specialist, and the CHR to ensure that the needs of begun members are optimally met. 
  • Works to create awareness, understanding, and positive interaction between Piikani members and programs of AKHS 
  • Works with other programs of AKHS to ensure that clients, families and communities are aware of and have accurate information realign to services available to them from the CHR and other local First nation communities. 
  • Providing Information and services on clean healthy water in Piikani Nation by weekly water monitoring and testing.  
     

 

Qualifications  

 

  • Minimum requirements, HCA or equivalent.  
  • Additional training in public health is an asset. 
  • Minimum of 2 years of experience in an administrative or reception role  
  • Must provides a drivers abstract 
  • Deep respect for and understanding of Piikani Nation culture, traditions, and protocols is essential; knowledge of the Blackfoot language is an asset.
