Compassionate Care at Home
The Home Care Program at Aakom Kiyii Health Services provides essential nursing and personal support services to Piikani Nation members who require assistance to remain safely in their homes. Our team works closely with clients, families, and healthcare providers to create care plans that meet each individual’s needs while respecting their independence, dignity, and cultural values.
Services are available to individuals recovering from illness or injury, those living with chronic conditions, and Elders who need ongoing support. Care is provided by qualified nurses, home care aides, and support staff who are committed to delivering respectful, compassionate, and culturally appropriate services.
Home Care may include nursing visits, personal care, medication support, and assistance with daily activities. The program also works with hospitals, physicians, and community services to ensure a smooth transition from hospital to home and to help clients maintain their health, safety, and quality of life.
Serving Nation Members of all Ages
- Monitoring health status for acute and chronic conditions and providing teaching and therapeutic interventions as needed such as management of diabetic devices
- Helping to assess, treat, and manage simple and complex wounds and assist with medical procedures.
- Providing foot care, particularly for clients with a history of Diabetes and cardiac/circulatory conditions
- Administering regular injections for the management of diseases such as those related to Arthritis and Mental Health
- Assisting with linkages to access community resources related to Health Care, and advocating for clients and families as they move through the Health Care System
- Providing Palliative Care/End of Life Care in the home to achieve symptom control and comfort with a holistic approach
- Assessing the need for medical equipment and assisting clients to access aids to help promote independence and quality of life
- Assisting with some personal care such as weekly bathing at the AKHS Health Center or in the client’s home
Intake & Screening Process
- Initial Identification of client’s needs.
- Identification of eligibility for service
