Position: Patient Navigator
Program: Home Care
Classification: Full Time – Term Position
The Patient Navigator will work directly under the supervision of the Homecare Program. The Patient Navigator will combine health knowledge, communication, and administrative skills to effectively assist in the delivery of Home Care services and adapt services to meet the changing health and social needs of Piikani members. Performs all duties and responsibilities in accordance with the Piikani Nation’s Personnel Policy & Procedures.
Duties include (but are not limited to):
- Assist AKHS clients with the process of accessing the RAMP program, AISH, medical coverage, proper identification, and any other requests as needed
- Assist and guide clients of Aakom Kiyii Health Services, to ensure they are receiving proper support and solutions as they encounter challenges within the health care system
- Collaborate with appropriate team members to ensure comprehensive care
- Follow up with clients to monitor progress and address any ongoing needs
- Advocate on behalf of clients to remove barriers and improve access to services
- Maintain accurate and up-to-date client records
- Provide educational resources and tools to help clients become more self-sufficient
- Assist the Occupational Therapist by performing tasks as assigned, recording, and reporting relevant data for interpretation
- Set up and adapt equipment such as changing wheelchair backs and adjusting wheelchair height as instructed by the therapist.
- Maintain Homecare equipment and supplies by methods such as cleaning equipment following established infection control procedures, maintaining inventories, and assisting with ordering supplies and equipment
- Complete required reports and documentation, competent in using electronic documentation and in maintaining statistical records
- Perform clerical/administrative duties such as follow up phone calls, booking appointments, creating charts, organizing and filing charts, relaying messages, recording data, organizing department environment
- Home, facility, and clinic visits with clients and families. Participate in Client/Family/Interdisciplinary meetings. Transport equipment and/or supplies to appointments as required.
- Respect traditional First Nation practice protocol(s) when engaging with Piikani members and clients
Minimum requirement:
- Post-Secondary health related Diploma/Degree
- Two (2) year of related experience is an asset
- Current First Aid/CPR
- Valid Class 5 driver’s license
